When discussing this possibility with the urologist, it is important to understand that the biggest factor underlying the success of vasectomy reversal is the length of time since the original vasectomy was performed. The more time that has elapsed since the procedure, the less likely that fertility can be restored. After an interval of more than 10 years, the number of sperm produced and the percentage of motile sperm may be significantly decreased with an increase in abnormal sperm. Moreover, even if sperm are restored to the semen, fertility is not necessarily assured. The reason is that the body may have created "sperm antibodies" which may prevent pregnancy even with a normal sperm count. Despite these caveats, vasectomy reversal is entirely possible. While microsurgical techniques have made it possible to reverse many vasectomies, conception after reversal cannot be guaranteed. Luckily, there are newer technologies to assist couples to realize their dreams of having babies. Even if the number of healthy sperm after vasectomy reversal is extremely small, or if sperm antibodies prevent fertilization, the pregnancy can be achieved through in vitro fertilization (IVF) and intracytoplasmic sperm injection into the egg (ICSI). Some men may even choose to forego vasectomy reversal and have their sperm aspirated from the testicle for use with IVF and ICSI.
