Getting By On Less Newspaper Column Ad Appearing 5-28-03 One benefit of hormone replacement therapy (HRT) is that it helps postmenopausal women avert the potentially debilitating effects of osteoporosis. Many women avoid taking supplemental estrogen, however, out of fear of experiencing side effects and an increased risk of more serious complications. New research shows that low-dose HRT combined with calcium and vitamin D supplements may increase bone mineral density (BMD) in women over 65 by at least as much as the higher hormone doses now recommended, without side effects. It should be noted that lower levels of estrogen (even with the supplements) might not be enough to overcome the drastic decline in estrogen that can lead to bone loss during the first few years of menopause. Women who use estrogens should be aware that there are a variety of hormones - some natural to women, some synthetic, and some of animal or plant origin and that there are different routes of their administration (oral, transdermal, intramuscular, or subcutaneous). The side effects and risks associated with HRT are different for different hormones and different routes of administration. For instance, oral estrogens may have an adverse effect on the liver and may cause increased intravascular clotting which may then lead to serious side effects. The most advantageous seem to be estrogens natural to women when not administered orally. However, individual indications and contra-indications, advantages and disadvantages, as well as risks and benefits should be considered when hormones are prescribed. HRT works well when used as indicated; however, it is not advisable for all women. Estrogen replacement is contra-indicated for women with personal or family histories of certain diseases and disorders.
